Please provide the CCC/IG# and investigative findings for the 8/07 DOVE
Academy falsely documented ten-minute checks, per the 8/07 QA review (see

1.04 Adequate staffing is provided to ensure the safety and security of
youth and staff. 0 P The program maintains the staffing ratios required by
the Department. At the time of the review, the staff to youth ratio was
sixteen staff to twenty-seven youth during the day shift and fifteen staff
to twenty-seven youth during the night shift. The facility exceeds both the
Awake ratio of 1:10 and Asleep ratio of 1:12 required by the Department for
Moderate Risk programs. A tour of the facility indicated that staff to youth
ratios is being exceeded. Video surveillance tapes were reviewed for the
night of August 2, 2007 beginning at 11 p.m. and ending on August 3, 2007 at
6 a.m. and on August 5, 2007 from 1 a.m. to 6 a.m. Video surveillance
revealed that on August 3 from 11 p.m. to 6 a.m. staff conducted ten-minute
checks on four out of the five dorms. Some staff exceeded the ten-minute
checks by conducting five and eight minute checks. Video surveillance also
revealed that the ten-minute checks were not being conducted in one dorm
(Sojourner Truth). The bed checks on this dorm were being done in varying
time increments, including every thirty minutes, twenty minutes, seventeen
minutes and fifty minutes. Video surveillance also revealed that on the
morning of August 5 the bed checks being conducted in Dorm Lucy Stone were
being completed every fifty-five to sixty minutes. The checks conducted on
the days indicated did not meet the standards outlined by the DJJ. Although
ten-minute checks were not being conducted, the check sheets falsely
documented that checks were being completed in regular ten-minute intervals.
